This diagnostic delay is one of the primary drivers of unnecessary suffering in perimenopause.
Perimenopause vs. Menopause: The Clinical Distinction
These two terms are often used interchangeably, but they describe different clinical stages with different management implications.
Perimenopause is the transitional phase leading up to menopause, typically lasting 4–8 years (sometimes longer). It is characterized by hormonal fluctuation — estrogen and progesterone may swing widely — rather than consistent decline. Cycles become irregular. Symptoms may be episodic and variable. Perimenopause begins on average in the mid-40s, but can start in the late 30s.
Menopause is defined retrospectively as 12 consecutive months without a menstrual period. The average age of natural menopause in the U.S. is 51–52. At this point, ovarian hormone production has declined substantially and symptoms typically stabilize in pattern, though they may intensify.
Postmenopause is the period following menopause. Without hormone therapy, estrogen, progesterone, and testosterone remain at low levels indefinitely — and the long-term consequences for bone density, cardiovascular health, and cognitive function accumulate over time.
Diagnosing perimenopause requires clinical history combined with laboratory assessment. A single FSH measurement is insufficient — levels fluctuate significantly during perimenopause. Hormone Therapy Options: What’s Available
Hormone therapy (HT) for women after 40 encompasses several different approaches — and the terminology has evolved significantly. “HRT” (hormone replacement therapy) has largely been replaced by “MHT” (menopausal hormone therapy) or simply “HT” in current clinical guidelines, reflecting the shift from framing it as replacing something lost to treating a clinical condition.
Estrogen therapy
Estrogen alone — without progestogen — is appropriate only for women who have had a hysterectomy, as estrogen alone in women with an intact uterus increases endometrial cancer risk. Estrogen is available in multiple forms: oral tablets, transdermal patches, topical gels and creams, vaginal rings, and vaginal tablets or cream (for localized GSM treatment). Transdermal estrogen is generally preferred over oral for women with cardiovascular risk factors, as it avoids first-pass liver metabolism.
Combined estrogen-progestogen therapy
Women with an intact uterus require progestogen alongside estrogen to protect the endometrium. Progestogen options include synthetic progestins (medroxyprogesterone acetate, norethisterone) and bioidentical progesterone (micronized progesterone, brand name Prometrium). Research suggests that micronized progesterone may have a more favorable safety profile than synthetic progestins — particularly regarding breast cancer risk and cardiovascular markers — though evidence continues to evolve.
Testosterone therapy in women
Testosterone therapy for women remains significantly underutilized despite solid evidence for its effectiveness in treating sexual dysfunction — specifically hypoactive sexual desire disorder (HSDD) and related symptoms. The 2019 Global Consensus Position Statement on testosterone therapy in women, endorsed by major endocrine and sexual medicine societies, concluded that testosterone therapy at physiological doses is safe and effective for women with HSDD. No testosterone product is FDA-approved specifically for women in the U.S. — meaning all use is off-label, but it is supported by clinical guidelines and extensive research.
Local vaginal hormone therapy
For women whose primary concern is genitourinary symptoms — vaginal dryness, discomfort, recurrent UTIs, urinary urgency — low-dose vaginal estrogen (cream, ring, or tablet) or prasterone (DHEA) suppositories address these symptoms with minimal systemic absorption. These are appropriate for women who want to address GSM without systemic HT, and are generally considered safe even in women with a history of certain hormone-sensitive cancers when used at approved local doses.

The WHI Study: Setting the Record Straight
No discussion of women’s hormone therapy is complete without addressing the Women’s Health Initiative (WHI) — the 2002 study that caused widespread abandonment of HRT and reshaped clinical prescribing for decades.
The WHI enrolled postmenopausal women aged 50–79 (average age 63) and evaluated a specific combination of oral conjugated equine estrogen and medroxyprogesterone acetate (a synthetic progestin). The study was stopped early based on findings of increased breast cancer, coronary heart disease, stroke, and blood clots in the hormone group compared to placebo.
What followed was appropriate for those specific findings in that specific population — but the interpretation was broadly generalized to all hormone therapy, in all women, at all ages. Subsequent re-analysis and decades of additional research have significantly revised this picture:
The elevated cardiovascular risk was concentrated in women who were more than 10 years past menopause when they started therapy — not in women who started HT near menopause onset (“the timing hypothesis” or “window of opportunity”)
Women aged 50–59 who started estrogen alone actually showed a trend toward reduced cardiovascular mortality in the WHI data
The synthetic progestin (medroxyprogesterone acetate) used in WHI is not equivalent to micronized bioidentical progesterone — studies using the latter show a more favorable risk profile
Transdermal estrogen avoids the clotting and stroke risks associated with oral estrogen, which undergoes first-pass liver metabolism
Current guidelines from the North American Menopause Society (NAMS), the British Menopause Society, and the Endocrine Society reflect this updated evidence. The Testosterone Question in Women: Underrecognized and Undertreated
Testosterone’s role in women’s health remains one of the most underrecognized areas of clinical practice. The 2019 Global Consensus Position Statement — developed by an international panel representing the major endocrine, menopause, and sexual medicine societies — concluded that:

For most healthy women under 60 or within 10 years of menopause onset, current evidence supports that the benefits of hormone therapy outweigh the risks when appropriately selected and monitored. The key variables are: type of hormone used (bioidentical vs. synthetic), route of delivery (transdermal vs. oral), timing of initiation (near vs. remote from menopause), individual cardiovascular and cancer risk factors, and duration of use. What is “bioidentical” hormone therapy?
Bioidentical hormones have a molecular structure identical to the hormones produced by the human body — estradiol, progesterone, testosterone. They can be FDA-approved (micronized progesterone, estradiol patches, gels) or compounded by a pharmacy per physician prescription. The term is sometimes used loosely in marketing contexts to include custom-compounded preparations that may not have the same quality controls as FDA-approved products. When a physician prescribes bioidentical hormones through a licensed compounding pharmacy based on individual hormone testing, this is a legitimate clinical approach — but the quality of compounding varies significantly between pharmacies.
When should I consider hormone therapy?
Hormone therapy should be considered when symptoms of hormonal decline are affecting quality of life, and when laboratory evaluation confirms the hormonal picture consistent with those symptoms. There is no single threshold — the decision is individualized. Most guidelines support initiating HT in the perimenopausal or early postmenopausal period for maximum benefit, particularly for long-term cardiovascular and bone health protection alongside symptom relief. Starting HT more than 10 years after menopause is approached more cautiously.
Does hormone therapy cause breast cancer?
The relationship between HT and breast cancer is complex and depends on the specific regimen used. Estrogen alone (in women without a uterus) does not appear to increase breast cancer risk and may slightly reduce it based on WHI and subsequent data. Estrogen combined with synthetic progestins showed a small increased risk in WHI — roughly equivalent to the risk associated with having one alcoholic drink per day. Studies show that HT attenuates menopausal changes in fat distribution and helps preserve lean mass.
